How to schedule an eHealthScreenings offsite lab appointment (if available).
- Health screening options, activity name, activity dates, and deadlines will vary by program. Check your program's health screening tile or Wellmetrics page for details.
- Note that this a standalone option. If you plan to submit the Health Provider Screening Form (HPSF), and your health provider sends you to an eHealthScreenings location for a blood draw, do NOT use the process outlined below, but visit the eHealthScreenings website directly. For instructions on using the HPSF, click here.
- Note that in some cases, offsite lab options may include Labcorp (an eHealthScreenings partner) locations.

Log in to your well-being program website. Locate and click the health screening Activity tile (tile names, activity dates, and deadlines will vary). On the activity description page, scroll down and Join/Enroll in the activity, if you haven't already. Next, click the Health Screening Options button.

On the Health Screening Options page, scroll down and click the Offsite Lab button. In the pop-up that opens, click the Schedule Screening button to transfer to the eHealthScreenings website.
- On the eHealthScreenings website, you will log in automatically via your well-being program account. If any required items are missing from your profile, you will be prompted to provide them. Once this information is filled in, click the Submit button.
- On the next screen, search for a convenient offsite lab using your address or the Using my current location option. Note that if you are using a Virtual Private Network (VPN), the Using my current location option may not be accurate. Be sure to scroll down to see all your options.
- Click or tap the Select button for the offsite lab of your choice. Note that offsite lab services are provided by Labcorp, an eHealthScreenings partner.
- You will now be prompted to read through and sign the screening consent form. Note that you must scroll through the entire consent before you can click or tap the I Agree checkbox. Then fill in your name and click or tap the Continue button to proceed.
- You will now be prompted whether you would like to schedule your appointment with Labcorp. Click or tap the Yes button.
- Use the Start Date and End Date fields to select a date range for your appointment, and then click or tap the Find Appointments button. Select a date and time that is convenient for you.
- The confirmation screen will display your appointment details, and you will also receive a confirmation email at the address associated with your well-being program account. Check this email for instructions, and the required lab requisition form to be signed and submitted at your appointment. The password to view/download/print the form is your date of birth, written as MMDDYYYY.
- To check your latest appointment details, or to reschedule or cancel your appointment, repeat steps 1-2 above to return to the eHealthScreenings scheduler, and click or tap the appropriate button.
- When rescheduling, your old appointment is automatically cancelled when your new appointment is confirmed. Note that if you reschedule an appointment, you may not receive an updated confirmation email, so always check the scheduler to be sure.

Click the chat icon (speech bubble) in the lower right corner of the page to connect with a live agent, or click the Submit a Request link at the top of the page, and we'll respond as soon as possible.
When using chat, be sure to close your browser window or tab to clear your chat once the discussion is completed. This will help protect your privacy, especially is you're on a shared computer.
When submitting a help request, please include the following information as applicable:
- Your name
- Email address associated with your well-being program account
- Your phone number and time zone (we'll need to speak to you personally for account, personal information, or protected health information issues)
- Your well-being program provider/employer
- Device you’re using and the operating system
- Web browser you’re using and the version number, OR Aduro Mobile App version number (when logged in, tap the menu icon in the upper right)
- Step-by-step description of the issue, including the names of any Activities, Paths, etc. involved
- Screenshots or video that illustrate the issue, if possible
