The Aduro® Human Performance Assessment (HPA) is an annual/periodic opportunity to assess your well-being in multiple areas of life, and see how it changes over time. At the end of the survey, you will get a report on your well-being, along with a lot of useful information and suggested Activities and Human Performance Paths you might find interesting. Availability and timing vary by program.
Q: Why should I take this survey?
A: Your well-being is of utmost importance to us. This survey will ask about a few different areas of your well-being. It’s an opportunity to assess your well-being today, see how it compares to others like you, and then see how your well-being changes over time. When you’re finished with this survey, you will get a report on your well-being, along with a lot of useful information and suggestions for Activities and Human Performance Paths you might find interesting. In short, this survey is the first step toward your goals! We’ll be here to support you each step of the way.
Q: How long should it take to finish this survey?
A: Give yourself 10 - 15 minutes to complete this survey. A little more if you are particularly thoughtful.
Q: What if I don't remember my cholesterol, blood pressure, or other health-related numbers?
A: That’s okay. Just answer “I don’t know” to those questions. You can pause the survey and come back and complete those questions later when you have the numbers handy.
Q: Is my data secure?
A: Yes. Absolutely.
Your privacy and the security of your information is our top concern. Our entire team at Aduro works tirelessly to ensure your data is safe and sound. We’ve outlined how we protect your data and activity in our Privacy Policy.
Q: How will my answers be used and who will see my information?
A: First and foremost, you get to see your data! At the end of this survey you will receive a report, telling you how your answers compare to a lot of other people like you. You can use your report to track how your well-being changes over time, identify new strengths, and see where you might want support.
Aduro uses your responses to identify which services you might find useful, such as educational or health improvement programs. We also use your responses to help us offer better programs and services over time. Your information may be disclosed to third party wellness providers Aduro has engaged with in order to provide you with relevant programs.
Your information may be disclosed to provide incentives and rewards for your participation in Aduro’s services.
Rest assured your specific answers will not be shared with your employer. Instead, your employer will receive “rolled-up” deidentified reports that masks your individual responses, providing a birds-eye view of what you and your coworkers need in order to flourish.

If you need assistance with your well-being program or program account, please submit a help request. Click the chat icon (speech bubble) in the lower right corner of the page to connect with a live agent, or click the Submit a Request link at the top of the page, and we'll respond as soon as possible.
When using chat, be sure to close your browser window or tab to clear your chat once the discussion is completed. This will help protect your privacy, especially is you're on a shared computer.
When submitting a help request, please include the following information as applicable:
- Your name
- Email address associated with your well-being program account
- Your phone number and time zone (we'll need to speak to you personally for account, personal information, or protected health information issues)
- Your well-being program provider/employer
- Device you’re using and the operating system
- Web browser you’re using and the version number, OR Aduro Mobile App version number (when logged in, tap the menu icon in the upper right)
- Step-by-step description of the issue, including the names of any Activities, Paths, etc. involved
- Screenshots or video that illustrate the issue, if possible
