Aduro® Connect Care provides personalized strategies, tools and impactful support to help manage and potentially reverse certain chronic health conditions, and is available to select well-being programs and members. Check your well-being program for availability.
Connect Care eligibility may be limited according to information provided during onboarding, to support member safety and align with evidence-based guidelines.

- Note to Bon Secours Mercy Health (BSMH) Be Well program participants: Be Well With Diabetes is a separate program offered through your program provider/employer.
Members are eligible if they have been diagnosed with any if the following metabolic health conditions:
- Type 2 diabetes
- Hypertension (high blood pressure)
- Hyperlipidemia (high cholesterol or high triglycerides)
- Cardiovascular disease (including heart disease)
- Overweight and obesity (BMI greater than or equal to 25)
- Prediabetes
- Metabolic syndrome
Members are ineligible if any of the following applies:
- Type 1 diabetes
- Currently pregnant
- Advanced kidney disease or dialysis
- Active cancer treatment
- Recent heart attack or stroke (past 6 months)
- Currently taking insulin
- Severe hypoglycemia (frequent or severe low blood sugar)
- Unmanaged serious psychiatric condition

- Email address associated with your well-being program account
- Your phone number and time zone (we'll need to speak to you personally for account, personal information, or protected health information issues)
- Your well-being program provider/employer
- Device you’re using and the operating system
- Web browser you’re using and the version number, OR Aduro Mobile App version number (when logged in, tap the menu icon in the upper right)
- Step-by-step description of the issue, including the names of any Activities, Paths, etc. involved
- Screenshots or video that illustrate the issue, if possible
